My guess is almost everyone has it, but some don't notice it.

Maybe they think the inner monologue is like a separate voice that you can't control, that narrates your life or something. I can't even imagine acting without thinking, while having total silence in my head. Yes I know about quick reflexes and silencing your thoughts during meditation, but those are different. But making deliberate actions without thinking beforehand? how the fuck?
