Some guesses of mine:

The shots were digital, artificial sound added to the video, after all none of the crowd moved even after the shots were fired until Trump fell down. Only then did they scream. As if on queue!

The gunshots were fireworks set off by them and when trump ducked behind cover the secret service members put makeup on him to simulate the look of blood on his face
