In most phimosis cases, the skin retracts partially so there´s enough space for pee to come out. I´m sure if its that bad it´s usually a surgical issue that is treated at early childhood by foreskin removal.

Phimosis is in stages, most people don’t have tight enough foreskins to the point they can’t even see their dick hole, they just can’t pull it behind the head
