It's not a semantics problem, it's an empathy problem. They don't fully understand, because they can't relate at all, even though they to some extent they realise what being an incel points to. For example, they will call you an incel as an insult, but will ask you "why don't you just get an escort then if you want sex so much", which obviously isn't the point of the former insult. Even if you did get escorts every day you'd still get look at as an incel.
