If you manage to get on a date one on one with a foid, but don't even hold hands, then maybe it means that you are not extremely ugly, or maybe it was an one in a million case, but you didn't pass the NT check, so you remain an incel. But just holding hands, means that she sees you as a romantic interest, so inceldom is lost immediately.
But in the "american" case you have made it clear that it's a date from the get-go. If it's just going out with one female, there's no certainty and maybe she could reject the guy's moves and claim that she only went out with him as a friend. I would assume so too, in my case tho there's a foid I've been on more than one "date" with but I'm still KHHV because when I tried to escalate or asked to meet at her place etc she blocked me so she probably just wanted attention and/or free drinks/meals.
