I think it means grounded in reality, even if the opinion isn't a popular one or commonly expressed or accepted by the masses. You're being based when you tell a truth or realize a reality that most don't acknowledge or talk about.

Or you're based when you act in accordance with reality, not what the hoi polloi tells you is the correct course of action.
