When we talk of it we think of a politician who has done wrong and a man may say "we must hold him responsible".
Some lines of thought:
We hold a man responsible for his actions.
We hold a man responsible for his words.
We hold a man responsible for his thoughts.
The idea is we hold a person accountable for what they DO, essentially, how they ACT and when they do NOT do what it is they are supposed to that is when we hold them account. But what is responsibility?

Here we find substance, we find as was mentioned "accountable and answerable". For whatsoever comes from our Thoughts, feelings and actions that is what we are held responsible for, but it is not linear in one answer. First from our actions, second from moral choices, when we allow wrong to occur in our presence we are held responsible for the good we didn't do when we could have done it.

We must be held accountable for what we do,
We are held responsible for what we do not do when we had to do it.
Therefore, responsibility is that which we are to do and must be done when not specified. What it is not is
Example of what it is: You must help another even if you might get hurt.
You must do your work otherwise it won't get done.
You must do the task which you are assigned.
You must do what is right, because it is the correct thing to do.
Example of what it is not: Leaving another, because you fail to own up to your actions,
Hurting another, because you hate yourself,
Blaming another for what was actually caused by you.
