When it comes to cousin marriages, its a matter of sharing recessive genes or not with your cousin, similar to how blue eyes are recessive, and thus almost always need 2 blue eyed parents to get them. Likewise, you'll only get defects if both cousins have that same recessive genes AND manage to pass it on, which is rare. Only about 2-3% more.

I have had the misfortune to go to school with and work with Pakistanis over the last 20 years and they are EXTREMELY inbred. You can tell by looking at a lot of them.
