They don't know. They genuinely think that looks don't matter. But true belief is exhibited in actions not statements. The way they act proves that they believe in the blackpill but they don't actually realize it.

For instance, it's easy to say "I am a Christian, so I love everyone on Earth" and a person may genuinely think that's how they actually are. However, it's obviously impossible to be an all-loving person. There are some people you might love (actually quite rare, I think), some people you like, most people you are indifferent towards, some people you dislike, and maybe some people you actually hate. Furthermore, people don't sustain these statuses for long (romantic relationships may turn from love to hate once the love chemicals in the brain run out). You can't prevent yourself from being human.
