Losers typically have a low "locus of control", a term for not believing that your actions can improve your life. It is extremely hard to get past learned helplessness once you got it. Learned helplessness is a result of serious, repeated, inescapable trauma in youth.
For instance, I have become a millionaire recently due to crypto trading but I still LDAR on my bed all day long, feeling like degenerate trash and a loser. Getting plastic surgery, to me, sounds like an enormous hassle. Merely arranging a rendez-vous with a surgeon sounds like a tremendous effort. The same goes for trying to meet women. I've taken anti-depressants, they do lift my mood but I remain the same lazy asshole nonetheless; I just become a happy bum instead of a miserable bum.
This is the ultimate blackpill: if your teenage years are miserable, you risk developing learned helplessness and then it's likely over for you. You'll become the stereotypical loser character in novels. The only thing I managed to do is stay clear of alcohol, but the temptation to go "full bum" is sometimes hard to resist.
