Serious How do I apply for SSI?

I honestly have no clue on where to start. I slept in till 5:00 pm today. Broken and defeated. Do I have to send in paperwork to verify that I am mentally ill? Is there a specific procedure that I have to follow in order to apply?

I know my chances are low, but this is really my only option.
