he's too far gone. his thyroid is fucked, absolutely no metabolism. if he lost the weight, he'd have tons of loose skin that would require potentially life threatening surgeries. the strain on his heart will probably cause a premature death even if he were starting to make a change. he's also clearly mentally ill.
