Reasons for leaving could be for various reasons, idolatry, welfare, criminal activity, shortages, hopes for a better life etc. Many North Korean defectors do eventually want to return, check out a documentary "Loyal Citizens Of Pyongyang in Seuol"
If you are a loyal citizen you can go to China or Russia to study
Defectors do get paid to say certain things. Certain things that did exist were absolved like public executions were removed over 20 years ago, and family punishment hasn't existed in 30.
