I'm gonna vote eye area because it is both an important facial feature (face is #1) and is the hardest to fix. Jaw might be slighly more important, but it's easier to fix. Mental health is underrated though.

Most important individual facial feature IMO. You can't ever be good looking with a bad eye area/midface plus it's one of the most tricky parts to get surgery on. It's more than over for eyecels.
Jaw/chin
This is important AF but you can still be at least a Chadlite with a weak chin as long as it's not really bad like extremely recessed. Also, fixed with surgery.
Ears
Not that important + easily fixed through surgery.
Nose
Medium importance, easily fixed through surgery.
Hair
Becoming bald is a serious blow on your looks and prospects with younger foids but not as bad as fucked up facial features. It's somewhat fixed by medication but the results are inconsistent and they come with lots of possible side effects, some of which we still know little to nothing about. Some people report taking them for ears with 0 side effects and great results, some other report crushing side effects + poor results. I personally decided not to take them, thus going from something like 4.5/10 to 3-3.5/10.
Skin
Really depends on how serious the skin damage is. The difference between mild acne/scars and cystic acne/scars is huge.
Height
Definitely below face but matters. Medium importance. Can't be satisfactorily fixed.
Race
Below both face and height. Low importance tbh. Can't be fixed with the exception of Michael Jackson.
Mental health
Least important item on the list in relation to sexual success unless it's something REALLY extreme. If you're able to communicate coherently here you can't be that bad, thus confirmed for least important item on the list.
