Not sure but if I were to take an educated guess I would say it’s a genetic roll of the dice.
My orthodontist brutally told me it's genetics about 30% of of population have it however your boy is being sent for an evaluation for surgery so hopefully I could ascend from chinceldom

like everything in the human body, I think it's partly genetic, partly behavioural because of soft foods. Paleolithic people chewed hard foods, raw meat, even used teeth to soften leather and didn't pull any wisdom tooth, same for american indians and all stone-age level populations
