It depends on the intensity of the desire as well as how 'orthodox' and 'vanilla' that desire is. Even a mild interest in something unorthodox in regards to sex can be considered a fetish, something like femdom, watersports or feederism for example.

If it's a paraphilia then it's a fetish.
