Imo besides sex a lot of incels are suffering because they are in a time of transition (has been ongoing fully since the sexual revolution of the 60s, sped up with proliferation of alpha redpill rap culture, pop culture, television and media/social media setting social norms since 90s to early 2010s) where society no longer needs unattractive males in any capacity while crazily demanding low status males carry out the provider and protector role nonetheless.
There's too many opposing ideas of what the meaning of life is. You are expected to follow a life script that doesn't exist anymore and are penalized if you don't live up to it. You have to keep reinventing yourself in a society primed for creative destruction and that caters more to the temporary whims of women than anything else while claiming to prize stability. Everything is out of whack.
