Obese women often form relationships with obese men because they find it preferable to being perpetually single. This is not because they inherently have higher standards than healthy women, it's because they perceive their value as being lower and are thus more willing to compromise. If a healthy woman becomes obese, her standards will likely fall dramatically.
On a post-apocalyptic desert island with one man and one woman, it's extremely likely that they would have sex regardless of their attractiveness as the woman would be unlikely to kill him in his sleep. The consequences of killing him would be to become as lonely as it's possible to be, unable to have more children and more at the mercy of nature (harder to get food, maintain a shelter, kill/ward off dangerous animals etc.) This extreme survival scenario would also promote the formation of a strong bond between them.
If a woman's fertility is running out and she has one viable (though not highly desirable) option, she will likely go for him as a man who cares for her and her children is very likely better than no man and no children, especially if the future is uncertain and potentially dangerous.
