There is a massive amount of deconditioning an incel has to put himself through when his parents told him all his life that he's very attractive and will be popular with foids in some near or distant future. The dissonance between the fiction he's been told from birth, and the reality of perpetual rejection that splashes onto his face like a bucketful of ice water, is too great to take in stride. Like the participation trophies of old, the child incel begins to see these lies for what they are, and the lies begin to have an effect opposite of their intention. That is, rather than boosting the incel's self-confidence, they instead lower it even further, both insulting his intelligence and reminding him of his decidedly substandard looks. But at the same time, there is still some kernel of delusion left in the incel's mind as the inner child clings to the once-accepted narcissistic axiom that the incel is exceptionally good-looking, both as leftover programming and as defense against the ego-crushing truth.
This is partly how so many incels develop concurrent inferiority and superiority complexes; an inability to look at oneself objectively that leads him to blame external factors for his plight rather than his own innate immutable flaws. It is the result of a sort of cult-like brainwashing butting heads with the objective and undeniable reality.
