somehow no one talks about it and there isn't even a story reason handwave to support it

you are just a troon instead of the original quake guy, and make putatively female (troon) sounds when you get hurt or jump

I was literally just about to post this.

You need to use a special, exclusive source port just to run it, but it looks pretty damn good. The mod completely overhauled the enemy/weapon roster with new skins and sound effects, and there are over 40 maps iirc. Goes way beyond what the other two installments did.
