Orthodontics with tooth extractions can really fuck up your face by removing structural support of the mouth, making the skin around mouth loose and making the mouth and lips horizontally narrow. The mouthpill is very obscure and it's not something you can really "excel" in: either you have a normal mouth or it's ugly. The actual size difference doesn't have to be too significant. In the example image, the guy on the right has clearly had orthodontics done on him and exhibits exactly the problems I described. The left guy looks pretty regular in the lower third area and is there just for reference. The size difference of the mouths isn't drastic but the difference in underlying structure and the effect on attractiveness is big.
