Never watched this before.

It's been a few years since I last saw Monster, but I think that Tenma is hiding from the police in a small village, after being framed in a series of murders, in said village he helps the local village doctor, he then meets a nice old woman that refuses to be treated despite being sick, and as she has an attack he proceeds to do an impromptu surgery alongside the local doctor. Then her son, who is a cop, walks in
This is pretty early on in the show
