I had a buddy that worked at my local water plant. I got a tour of both sides of the faucet. Water in - from the river and treated. And water out. Cleaned sewage water going back in the river.

Dude, before it gets treated its got so much weird shit in it, its anyone's guess. But most REAL sewage is in sealed pipes. Open street stuff is just runoff and far less horrible. Ofc, thats in the states. Not sure about other places.
