If he gets a jaw implant, he'll have a horse face because he has a long philtrum. His eyes are asymmetrical, which can't be fixed, and a nose job won't make him attractive either.

He's not holding his head straight. He's got issues but the nose is by far the biggest and this can be fixed.
He can go from ugly to neutral.
