I remember a really old game years ago in which your character is at a party and the objective was to annoy people by being as much of a douche as possible, like ripping ass, stealing people's beers, puking on the furniture, etc.

In my hypothethtical game, it would be something like a stealth-based game similar to the Metal Gear series or Manhunt. However, you are given missions of different locales in town and the objective would be to vandalize them in the most disgusting ways possible.

I.e. upper-decking all of the toilets, heating up turds in microwaves, vomiting on the hoods of parked cars, etc.

As you do this you have to avoid being caught by patrolling security guards, cameras, or bystanders and part of the fun in the game would be watching NPCs reactions as your "creative pursuits" are discovered as you watch them while remaining hidden.

The controversy would stem from how over-the-top the game would get with its gross-out humor.

You had one of the best ideas.

It would be awesome if you could start in an open world and have a leveling system. First you will prepare buckets of water to fall on people who are going to work, so you will have more points and you can go further in terms of stricter zones and higher level people. And finally you can set traps that almost kill people who are unlucky enough to run into them. You may also be chased by the police, and you can become famous for it.
