Always wondered this myself. Plenty have underage girls in their ads but ads are usually just teasing and clothed so it is okay-ish
But then you get stuff where there is a blowjob video from a 16yo that stays up for months/years and even if you report it nothing happens. How, then, do these sites not get taken down?
