It's fiction. They can write Quagmire to be anything they want. The characters were created in the 90s too, dating back them was better by a huge margin and a guy like Quagmire could choose if he wanted to slay or form a family.
A better question is in Seinfeld how could George Costanza date exclusively attractive women that are much taller than him. He never once dated a short ugly woman.
He's always losing those women, though. If his parts showed him just failing (which is what would happen in real life) it would've been incredibly boring.
He's a pilot so he's moneymaxxed and statusmaxxed. Also he has chad tier charm and finally there's no indication that he's ugly, that's just the goofy style of the cartoon. But I do think it's funny that he's a 60 and his entire character is just fucking young foids including underage foids. Gigachad character honestly.
I haven't watched Family Guy in years other than reruns of old episodes but from what I know they've basically neutered his character now because he was too based for modern tv.
