This is how I'd make the "perfect" RPG if I somehow had the resources to do so.
• semi-open world, but linear progression is required.
The goal of the game is to go on a journey through a world and reach an objective.
As you travel throughout the world you'll eventually come to face dungeons or fortresses you have to engage with to progress. Think of a mountain that's unclimbable and requires you to delve into an underground labyrinth to come out the other side.
• starvation
Survival wouldn't be the main focus of the game. However, I would include a hunger mechanic for the sake of immersion. You'd have to eat every so often to retain your stamina and not starve to death. I'd make it so that food was plentiful so you wouldn't actually have to worry about starving. However, in some dungeons or labyrinths, food might be scarce, so it would be essential to preserve items and stock up on them beforehand.
• combat
The sword combat would be more immersive with greater control of what your sword can do. Think Skyward Sword.
Enemy AI would be programmed so that if one move hurts it, it will block said move if you try it again to prevent spam and encourage you to play with more variety. Likewise, if one of its attacks hurts you, it will spam the attack until it stops working.
• NPCs
Each NPC will have 5 different schedules that will be randomized to make them appear more alive. Assuming I had infinite magical resources to make this game, I'd try to implement a more advanced AI that has completely randomly generated dialogue. You'd be able to kill, eat, and rape NPCs. You'd also be able to enslave them.
Some would give you quests and reward you with resources or sex.
• side content
The world would be filled with monsters and dungeons. Let's say you're in a forest. There's a change a randomly generated monster will spawn. To defeat monsters you have to dismember them. Monsters are somewhat easier to defeat than humanoid enemies. However, they also deal more damage. Let's say you're fighting against a long necked creature in the middle of the woods. You can instantly decapitate it if you time your attack right. However, it can also instantly kill you.
Killing monsters will reward you with special materials that you can either trade in for gold or use to construct special potions and in some cases armor.
• weather system
There would be a weather and seasons system. Weather based on rng. Seasons are based on an in-game calendar. Both would affect survivability. However, there would be magic that allows you to bypass the effects of cold for example.
Seasons would affect which monsters you face in the world.
• story
The story would be fairly simple.
1. The skeleton king.
An ancient king tried to become immortal and bypass his illness by prolonging his life by absorbing energy from human sacrifice. He was cursed by the gods to forever be immortal in the form of a skeleton. You must journey to his castle and kill him.
2. Ancient being
This one is similar. An ancient being resembling a fantasy elf with a long neck has prolonged his life by absorbing energy from human sacrifices. You must journey to his castle and kill him.
3. Dragon
A good old-fashioned dragon.
The dragon is a sadistic demon who is torturing the world by creating monsters and killing people randomly. There would be a mechanic where he could randomly burn down any village and kill all the NPCs. The dragon would also be capable of raping female NPCs with a giant spiked penis. In the final fight, you can kill him by baiting him with a dead female NPC and chopping off his penis.
