From all the videos I've watched, it's usually just down to whoever draws first. That's why if you're confident that you may need to defend yourself from an aggressor, it's actually ironically more effective to make yourself look more feeble and scared than you actually are. It puts them at ease and makes them think they can just push you around with no consequences, which therefore makes it so they take far longer to react when you drop the facade and set out to take their life.

If the person doesn't already have a gun in their hand, the sheer shock of being shot will make it so that their brain panics and they'll ultimately fail to even draw it before they're fatally wounded and incapacitated.

Being in an actual duel sucks—it's hard to hit the other person while also avoiding being shot yourself. You have to get really lucky and manage to hit them somewhere that puts them out of action while praying that it doesn't happen to you first. Even if you mortally wound them (a shot to the neck, for instance), they're still going to be able to fight back and make rational decisions for at least thirty seconds before finally biting the dust.
