In 1999, the first case of a perianal lesion located at 12 hour, was reported on a twelve years old boy, initially suspected of sexual abuse, caused by a German shepherd dog. This report was based on the morphological, serological and molecular confirmation of dog semen.9
In 2009 the case of a twelve years old boy with lesions made by the anal penetration of a Bulldog dog penis (located at 3, 6 and 11 hour) was reported. The diagnosis was based on the child’s telling, investigation of the site and the finding of canine semen on the victim’s clothes.8
In both cases previously reported, the children admitted to have sexually stimulated the dogs before penetration. In the present case, this seems highly probable, since the girl was playing alone with her pet and there have not been reports of dog raping humans. Moreover, dogs have no natural inclination to breed with humans.8,9
The present case corresponds to a girl younger than the previous ones, (six years and two months old) and involves a different canine breed (Labrador retriever).
Sexual games are frequent at the stage of psychosexual development of the girl at the moment of the event. That stage is characterized by the interest in the difference of sexes and in the external genitalia.
The tear location (hour 1) is not the most traditional of sodomy10 , although it is consistent with the lesion reported by Wiegand (1999) and with one of the lesions described on Blevins’ report (2009).8,9
The canine penis anatomy and copulation physiology may explain the lesional patten found. Indeed, the complete erection of the dogs penis occurs after penetration (triple its volume), which prevents the separation of the female without generating vast lesions, as those suffered by the children in the three reports. 11
In our case, as a sign of minor significance, but of value in the context of this presentation, it is important to mention the superficial erosions on abdomen and external face of the thigh, compatible with those made by the front paws nails when holding the girl from behind.
We do not know if dog semen was not found due to insufficiency when taking the sample, or because the dog did not achieve ejaculation
In any case, the complete case assessment by the healthcare team, including the inspection of the site, the paediatric follow up of the girl and her family during 8 years, and the subsequent re assessment by an independent forensic psychiatrist, allow to confirm the diagnosis of anal tear due to the penetration of a dog’s penis.
