It swings more like lymphedema (pouch of fluid) than fat. When your BMI goes >50 your body can be inflamed enough to cause some lymph nodes to clog up and over time lymph fluid accumulates.

Sometimes the inflammation lessens enough with weight loss (e.g. through gastric bypass) that it drains and shrinks by itself. Otherwise she would need to either see a vascular surgeon for a lymphatic bypass or a plastic surgeon for direct excision.
