That's something which needs to be investigated. There are a few sex mods for games like Sims and Skyrim which could serve as initial boilerplate. Or also some other ideas. For example, sex is usually connected to rhythm, so combining it with a rhythm game could make sense. Instead of looking at colors flying at you, you can look at nude body parts requiring sexual attention, for example.

Or having "proper" simulators which IMHO seems to me the most obvious gameplay mechanic: Simulate brothels (or "sexual amusement parks"), simulate managing a harem (where e.g. each member has mostly sexual needs instead of "normal" needs which need to be fulfilled) etc., simulate a porn studio.

All of that could be combined to make a giant AAA game with purely sexual content if you had the budget: You make a brothel of "hypersexual" girls where you can deploy various sex toys in rooms and you need to manage their sex life because they can't. You have to balance making them fuck each other in ways they enjoy, taking clients where you have to not only strategically decide which girl to apply to (so the client enjoys it but the girl doesn't loose too much sanity), but also how to fuck the client and execute it well in a rhythm game and shooting porn where you have make a video which fulfils some people's fantasies. But even single parts could, if well executed, make for a decent gameplay IMHO.

I think the genre can grow over time, the first shooters also weren't that great.
