Meh. I prefer Garry's Mod. Much more weapons and you can get a gore/dismemberment mod from the workshop.

You can create your own mini ER scenarios in pretty much anywhere you can think of, whereas in GTA V you're limited to whatever is in Los Santos. I will say that GTA V is more realistic, given there are cops to run from and the NPCs don't stand in one place.
