I used to troll pro-ana communities a lot and my conclusion is: most of the girls are either autopedophiliac (i. e. being a little girl fucked by an adult is their fetish) or they used to be fat and bullied (I'm using 'bullying' loosely here; the bullying a femoid can experience is nowhere near the real bullying), they fell for the skinny meme and took it to the logical extreme. For the first group, getting insanely skinny is a cope that helps them reverse ageing in their minds ('I have the same weight as I did in middle school') and also enhance the fragility/innocence aspect which plays a huge part in the fetish/paraphilia.
