Many reasons. High sugar diet, acidic diet like soda and fruits. Even diet soda causes acid erosion.
Also some medications cause a dry mouth which is bad for teeth. Some people also may have a dry mouth naturally.
Another reason can be acid reflux.

Teeth can also fall out if you have bad gums which you can get for many reasons too.

Even if you have a good diet they can rot away eventually because it is common for old people in my country to have dentures. That may be because they didn't brush them for years even though they had a good diet.

Some people develop plaque easier than others so if you have good teeth without going near a dentist it probably means you have food genetics in that regard.
