Genetics. I had immense back pain by the time I was only 20kgs overweight. I went to the doctors and they told me it's due to excess weight (my back can't support even a few more extra kgs JFL).
There are people over 40kg overweight who have practically no back pain.

The thing is, I asked them if the issue was bigger, because I didn't think just 20 extra kg would cause back pain, and after examining me, it WAS the weight.
It's just that my spine can't support more kg.

look alot of drs simply lie. if u have seen only 2 or 3 drs chances are they want free money they see you for 30 secs dont do any test dont try to listen to you then tell u any bullshit they want.

im not a dr but im 90% sure its not the 20kg weight no matter how weak you are being 20kg overweight wont give you backpain no way in hell this is the real diagnosis

We have free healthcare, I didn't pay for any visit and I visited 4 doctors.
