I would have mentalcel be defined as someone with the Strong Possibility of escaping inceldom if it weren't for their psychological issues.So it really boils down to:

1. Normie-tier looks rating
2. Mental illness

So, in response to your first question. An ugly guy with serious mental issues can be a mental-cel by going with (2) definition. However, he fails (1). This individual would be labeled an incel with mental illness.

As for individual in the second question, someone who is a sociopath and with low IQ does qualify for mentalceldom if said individual has normie-tier looks (according to my definition).

Quick hierarchy of possibility of escaping inceldom: Mentalcel > Incel > Incel with mental issues.

Can't, too many doxxers. I have/had very limited appeal IF I was tanned and looksmaxxed (well dressed, and my skin not suffering breakouts or dryness common of nerdics).
