Just skip the retinol BS and go straight to the good stuff—tretinoin. The reason why I say this is because retinol isn’t regulated (there is no retinol monograph) and hasn’t been proven to be effective (all of the studies have been extrapolated from tret). Retinol has to first convert to retinaldehyde then retinoic acid for it to bind to the skin receptors and work. If irritation is your concern there’s lots of ways to mitigate the side effects.

Do you have a primary care doctor already? Call their office and ask if they can prescribe it or if they would refer that out.

Whether or not you need a referral depends on your health insurance, I believe. If you have a document from when you got your insurance that says what is and isn't covered, it's probably in the under "specialists" or "specialty care". If you have insurance through work, you can ask the person in charge of insurance (usually HR).
