Happiness is relative, a starving African who received an extra loaf of bread one day will be as elated or more so than a Chad who received a love confession from another guy's girlfriend, or a hedge fund manager who profits an extra $50,000,000.

One can have the worst life possible but still persist for years on account that that person believes conditions will improve, things will get better, there is a chance for escape, to be free.

The blackpill is so soul destroying because it reminds you that there is no hope. The species has survived using mechanisms that reward the genetically blessed with endless pleasure, and the genetically cursed with endless pain. An incel in the past could cope with religion, patriarchal matrimony, statusmaxing in an era where status meant everything, and cope with his ugliness because he knew no better than to buy into the hope he was offered.
