On some grand righteousness level or whatever, I'd consider option 2 to be the "right" option because 5>1, like if you have equal 2 tribes of cavemen, where the only difference is tribe A is all cavemen who would choose to let 5 tribesmen die, and tribe B is all cavemen who would chose to sacrifice 1 for the 5. Tribe B would be more likely to survive and subjugate tribe A because they have more manpower. So option 2 is "right" because the way of thinking and its consequences offer a higher survival rate to the tribe than option 1, and at the end of the day, in nature, it only matters if you're alive and have the means to stay alive. I look at it from a tribe perspective because we are a social species.
On a personal level, option 2 is what I like to imagine I'd choose, but I can't really guarantee it because you can't really know what you'll do in a certain situation until you're put in it. It's not because I care about making the "right" choice or whatever, my reasoning is: option 1=chance of 1 person owning a favor and 5 families and groups of friends potentially pissed off, option 2=chance of 5 people owning me a favor and 1 family and group of friends potentially pissed off. Higher potential reward, lower potential loss. Kinda pointless taking this literally though since irl there would be a lot more factors to consider.

I'd guess because you're killing someone who "isn't involved", so it means for example that you're more willing to kill a random person and harvest their organs if it means saving 5 people (e.g., China organ black markets).
