When you have a long midface and your bodyfat is low, it makes it look longer because your face is more gaunt. Facial fat adds width/fullness to the face.
I have an extremely long midface as well, so I understand this problem very well. I got called horse face a lot in high school. I was a gymcel in my late teens and in hindsight, it only made me look worse. Kind of ironic that back when I was super lean and ripped, I looked my worst.
Most looksmaxxers dont understand these sorts of things though. They think being super low body fat and having almost no facial fat at all looks good. I think there's an aspect of male gaze when it comes to this. Also having a gaunt face makes you look haggard as fuck when you're past your mid 20s, regardless of facial shape.
Anyway I've had many sessions of fat grafting to my face so I can be skinny without having a gaunt face. I'm getting another session in a week.
Long midface is literally the worst facial flaw you can possibly have because there's no way to shorten the distance from glabella to subnasal. Not even the lefort surgeries do this. When you have a long midface you pretty much need to maximize every other feature you possibly can, which will likely require a lot of surgeries, and some of those surgeries will need revisions. And in the end you'll still have a low ceiling. I've lurked plastic surgery communities for many years and the guys who get the most benefit are the ones with compact faces but they just have a weak chin or a bad nose.
