It's because they only see the surface, what's right at front of their eyes. Just like how lookism works, they only care about what they can see, not what they can/should know if it doesn't please or help them. Does anybody really ask to themselves who the hell invented toilet paper or the microwave? No unless you ask it to yourself out of boredom/curiosity, the latter being a trait which is more present among smart people (yes it is) and is also related to emotions and need. So does knowing about the blackpill make us somehow smarter than the avg person? Possibly, but why we even got to know it in the first place is because we started to question the surface out of personal experiences (again, emotions and need are key) without them we would be just as unaware as NPCs are.

tldr ngltbh; most humans lack interest in things that they can't see if it doesn't help them, the blackpill being such an unhidden but obvious truth at the same time displays this.
