It's shitty advice, but you can't say anything back to them because it's assumed that therapy is actually legit. So the person who is told to get therapy is unable to say anything back. It's just like when normies insult you and say "I was just joking bro"
 
I'm sure they mean it as legitimate advice, it's just they don't want for you what you want for you.

I agree with OP, "get help" is just another way of saying "you're nuts".
 
It’s not really an intended insult because people are who say that actually this k that therapy works. For me, whether something is insulting or not is hinges upon whether it was said with intent to cause offence.
