I believe that the ever present dread that we experience as incels doesn't so much stem from not having physical access to a woman and going on decades without physical touch but rather from not having a clear pipeline to attain and keep a woman.
A perfect example is how Muslims/Middle Eastern societies deal with relationship pairings.
Generally there is no dating and Men and Women are arranged to be married at birth and only then can the man mate. There's also generally a bit of an age gap with the man being older than the woman. I distinctly remember some British tv program where there was some right wing Presenter who was interrogating a 27 year old dude from yemen who got arranged married to a 13 year old girl. The Presenter was absolutely disgusted by the guy even though he was just born into this system. ( though I get being mad that the Arab guy immigrated here cause he should keep sand people shit to himself.)
Arab men are Virgins until marriage, which can be until their late 20s to early 30s but they aren't depressed like Western men in the same position. The reason is that. They have a system in places that will get them a wife and that wife is assuredly going to stay with them until death. If we had this system then incels in the west would be long gone.
