I read this awesome comment on why it is so fucking hard to get help in the US

Largely its' put on the clinician to take clients who are willing to pay full fee (indicative of privilege or "worried well") and use a sliding scale to take a selection of clients for low or no fee. Imagine clinicians in orthopedics or another specialty care service line if they were getting reimbursed crumbs for their time and had to take a sampling of no/low fee patients. This raises the threshold for individuals to even think psychotherapy is an option, and if they have do attend session, its 4 sessions of CBT for their presenting state - cutting off just in time to do real work on their internal psychological disposition."
