There was a surgery that could restore your healthy state with a 5% probability. In 95% of the outcomes you will die in the surgery, pretty much instantly.
Will you take the 5% chance to live a much better life or will you cling to your survival no matter how much pain you will have to endure?

Like everybody else and since I was 18, my brains is slowly destroying itself. Plus I'm already mentally ill. But to answer your question, I personnally would not do the surgery, because I have chosen that I wouldn't to heavy surgery at all. I have zero reason to stay in this life, so I won't try to survive at any cost. If the pain becomes too loud, I have precise solutions to make them stop forever, instantly and with no pain.
But it's only my personnal choice, while another guy would try to restore his health despite the few chances.
I don't understand that type of thinking at all. I think it might come from a deep fear of a potential afterlife? Or maybe it is simply a strong survival instinct, to survive at any cost, no matter how that life will look like. I think you can defeat that with rational thinking.
Imagine being sent to a battlefield for a war you don't want just to save the interests of rich people from your country while normies continue to live normally.

It is an extremely strong survival instinct, that is "programmed" in our brains. We invented afterlife judgement beliefs to establish this survival socially. In fact our brains are ready to do anything to survive, even modifying themselves. If a very huge trauma puts you in danger of killing yourself, your brain can decide to erase the trauma from your memories. The brain can even invent memories, based in memories of other individuals.
Our brains are survival-machines at any cost. If your brains estimates that killing someone saves you from committing suicide, be sure the brain will make you kill someone, just to stay alive himself. And the brain doesn't try to survive "on a long-term", no, he does that on a very short term. He tries to gain living time, moment after moment.
The only means to avoid being manipulated by your own brain is to take clear and very strongly thought commitment. Like refusing ANY huge surgery. Or to act or die for a cause.
It's crazy because I completely lost that survival instinct. Like I wouldn't want to feel pain. But the thought of simply not existing is the most comforting thing I can think of.
