Literally nobody is claiming finasteride never causes side effects. The picture is "debunking" a made up argument, in other words a strawman. ED and gyno are well known to be somewhat common side effects.

Look, I've been on finasteride for a little over 10 years now at this point. Does it have sides? Absolutely. I got gyno and had to get surgery for it. But had I never gotten on finasteride, I would be NW5 or worse by now.

I used to be active on hair loss forums in the late 2000s, and there were plenty of guys like that on them. They were the most vocal when it came to talking about how horrible it is to go bald but then complain endlessly about potential side effects of finasteride even though they never took it. They'd make thread after thread linking to the absolute worst cases of finasteride side effects they could find and generally crank the fear mongering up to 11.

Then you had the melodramatic users who took finasteride for like a month....sometimes a week and sometimes even a day and swore that they had a whole slew of side effects long after they stopped taking it. Everything from "brain fog" to ED to depression to weight gain. Some might have just been trolls but the majority genuinely seemed like they truly believed that finasteride completely ruined their lives beyond repair. There even is (or at least was) an entire forum dedicated to people who had (or at least claimed to have) permanent side effects from fin long after they stopped taking it. I'm sure it was mostly the nocebo (antithesis of placebo) effect, or exaggeration in the very least.
