Don't keep putting it off and letting the prospect worry you. I have had similar struggles and some times it went ok, sometimes it was humiliating, but it never was bad enough to do much additional damage compared to what came before.

Also, in my experience lying to the doc is also a bad idea in general. Once the shameful truth is out, you are actually really you in the conversation and there is a limit to how mean and offensive a professional can be.
